The Fine Arts & Activities Eligibility form needs to be submitted to the Activities Office once per school year. If you have already turned in the Athletic Eligibility form, than you do not need to turn in the Fine Arts & Activities Eligibility form.

 

The form is found in the Wayzata Epay store. You will need to have an Epay account to submit the form.
