Change of Enrollment Notification

Below is the Wayzata Public Schools change of enrollment notification form that should be filled out by parents only if their children are:
  • Moving out of Wayzata Public Schools
  • Moving to another address within the district
  • Will not be attending Wayzata Public Schools for any reason during the next school year
